  1. State the background and the question to decide
  2. List comparable options and constraints
  3. Link evidence, risks and unknowns
  4. Name the decision owner and the input deadline
  5. Record the conclusion, the reasons and when to review it

Keep it traceable

Don’t save only the final answer. Keep the options you dropped, the key objections and what was known at the time, so the team can later understand why the decision made sense.
